amr: remove shift out of the AMR_BIT() macro.
...
MSVC doesn't like the offsetof(..) >> 1 construct, it interprets it as
a non-literal, thus causing use of this in static tables to fail
compilation.

Remove explicit filename from Doxygen @file commands.

Passing an explicit filename to this command is only necessary if the
documentation in the @file block refers to a file different from the
one the block resides in.
